Innovis Health wants you to fully understand your bill. That's why we go above and beyond to walk you through the billing process and help you understand exactly what is on your bill, what services were performed and what portion you are responsible for paying. You may receive two separate statements for your visit to Innovis Health. Billing statements received from Innovis Health Clinic are for services provided by Innovis Health’s professional staff which includes professional fees and some ancillary services provided by Innovis Health Clinic. Billing statements received from Innovis Health Hospital represents a facility fee only. The hospital statement accounts for nursing, room/care, lab, radiology, as well as other ancillary services, that may have been incurred during your visit/stay at Innovis Health. Payment for all services may be made with cash, personal check, cashier’s check, money order or credit card (Visa, MasterCard, Discover).
Payment Guidelines
All accounts less than $500.00 must be paid in full within 30 days unless other arrangements have been made. Innovis Health’s minimum monthly payment policy is 10% of your balance or $75.00, whichever is greater. 100% of all estimated charges for Cosmetic procedures will be required prior to surgery. Innovis Health wants you to know that NO ONE will be denied emergency services at Innovis Health based on their ability to pay. Programs are available to assist in developing a payment plan that will fit in your budget.
Financial Counseling
At Innovis Health, it’s important that you receive the care you need, when you need it, regardless of your financial situation. If you are concerned that you may not be able to pay for healthcare or you do not have health insurance, we may be able to help.
Innovis Health has financial counselors on staff that will assist you in exploring ways to manage your healthcare costs. Some of the ways that our counselors can help are: Medicaid/Medical Assistance applications and eligibility, Social Security Disability Applications and pharmacy/prescription assistance. Financial assistance programs, including discounts that are based on your income, assets, and needs along with a variety of manageable payment plans, including interest-free and long-term financing, are also available through our Financial Counseling Department. You may have health insurance that covers all, or a portion of your bill with Innovis Health. Although you are ultimately responsible for all charges incurred during your visit to Innovis Health, we have agreements with many insurers and governmental agencies that authorize us to submit claims on behalf of our insured patients. You must present your insurance card(s) to the receptionist each time you register for a visit, and any co-payments and possible deductibles required by your insurance company must be paid at the time of service.
If your insurance company has not processed your claim after 60 days, Innovis Health suggests that you contact your insurance company. Should your insurance company require another claim or additional information, Innovis would be happy to furnish your insurance company the needed information. If your insurance company fails to make payment or denies coverage, all or in part, you are responsible for the prompt payment of the entire amount owed to Innovis Health.
Estimates
The fees for your hospital and/or clinic visit will vary according to the nature and extent of the services rendered. Patients may obtain an estimate of fees on our price estimator page or by calling our price quote line 800-437-4054 ext. 8739 or 701-364-8739. Please leave a detailed message on what price quote you are requesting and we will be happy to research and call you back with in 72 hours.
Billing Disputes
Please contact our Clinic and Hospital billing office with any questions, concerns or disputes regarding your bill. If additional information or consideration needs to be taken in a billing dispute, the information will be forwarded to the appropriate individuals for review, consideration and explanation. If you would like an itemization of charges during your stay, please contact us and we will be happy to mail out a detailed statement.
Preauthorization
Preauthorization is required by most insurance companies before services are rendered. Contact your insurance company to verify if authorization is required. Failure to obtain either preauthorization or a second opinion may result in your insurance company refusing to pay your claim. You may be held responsible for the billing.
Referral
If you are seeing a doctor who is out-of-network, you are responsible for obtaining the approved referral from your primary care clinic or primary care physician. Failure to do so may result in reduced benefits or no benefits from your insurance company, and you will be responsible for the billing.
Privacy
Due to federal privacy regulations, the Innovis Health business office may not always be able to answer all of your questions regarding your spouse and/or other family members. In order for us to do so, Innovis Health requires that an authorization is obtained with the patient's signature. Please refer to the “Forms” link for our authorization form.
