A New and Better Way
Innovis Health Jamestown is proud to offer a Medical Home approach to your child’s care. A Medical Home is not a building, a house or a hospital. It is not a place, but an approach to health care for kids with special needs.
In a medical home setting, the primary medical provider partners with a family and patient to improve health outcomes and quality of life for children with chronic health conditions and disabilities.
The primary physician coordinates a team to make sure that all medical and non-medical services needed are received.
All care is family-centered in a medical home. Information and concerns are shared with everyone who is caring for the child. A medical home promotes parent-to-parent connections and networking with other families.
